Eligibility for DVA Exercise Physiology and Physiotherapy

If you are eligible for DVA treatment, currently hold a gold card or white card, or possess written authorisation, you can access vital clinical care interventions for chronic disease management and recovery from significant illness or injury.

Intervention for Excercise Physiology

We prioritise transparent communication and collaboration throughout the intervention to assist our DVA-eligible individuals in achieving their wellness goals and improving their quality of life.

Referrals

Accessing our exercise exercise physiology and physiotherapy services requires a referral from a healthcare provider, such as:

  • Local Medical Officers (LMO)
  • General Practitioners (GP)
  • Medical specialists
  • Another exercise physiologist transferring the entitled person
  • Hospital discharge planners
  • Treating doctors in hospitals

Referrals should be documented on a DVA Request/Referral Form (Form D904) or official practitioner's letterhead, including essential details such as:

  • Name of the entitled person receiving services
  • DVA file number of the entitled person
  • Referrer's healthcare provider number (or hospital's provider number if discharge referral)
  • Date of referral
  • Condition to be treated

Ensuring all necessary information is included allows our team to provide a seamless and efficient process for accessing our services.

  • Long periods of sitting, especially on non-ergonomically designed chairs
